The Diabetes Team have supported an innovative programme funding health inequalities projects across Cheshire and Merseyside and Lancashire and South Cumbria. They are really pleased to be able to share with you all a dynamic, interactive online report which has been created.
The report highlights the Diabetes Health Inequalities Programme (DHIP) which focuses on 16 community led projects aimed at improving the care of patients with diabetes in harder to reach communities. Within the report you can read about why and how they supported this Programme, project details and their outcomes and downloadable resources.
